• Parts of the form with the asterisk (*) sign are mandatory. Make sure they are completed.
• Make sure you complete the box below the form with the letters displayed below before you submit this form.
• All attachments to be uploaded with the registration form should be in JPG format.
• Scanned document, ie JPG file(s) to be attached to the form should not be more than 400kb in size.
